Get Ready for the Season with a Free Sports Physical!
You’re working hard to prepare for the upcoming sports season, but don’t forget—before you can step onto the field, you’ll need a sports physical. The Center for Orthopedics is offering quick, easy, and free physical examinations.
Event Details:
- Date: Friday, June 13, 2025
- Time: 1:00 p.m. – 5:00 p.m.

How to Participate:
- Visit your school website to download the 2025–2026 Ohio High School Athletic Association (OHSAA) Preparticipation Physical Evaluation Consent form.
- Complete and sign this form with your parent or guardian prior to your exam, or bring your parent/guardian with you. Forms will also be available at the event.
- Bring any relevant medical records you may have.
